The application process is very simple.
Just follow the three steps below and we look forward to seeing you soon.
All applicants applying for the first time must apply in person.
Determine If You Qualify
Applicants must meet ALL of the following requirements:
1. Adult Age
- Medical – 18-64
- Dental – 18-65+
2. Employed
You must have a paid job OR have a household income and perform an important unpaid job, such as:
- caregiver
- volunteer
- attend school
3. Uninsured
If you have any type of medical insurance, you are not eligible for medical services at Bethesda.
If you have any type of dental insurance, you are not eligible for dental services at Bethesda.
Apply In Person
To become a new patient or to renew, come visit with an Admissions Advisor at the clinic.

Admissions is open on the following days and times each week:
Monday 9am – 12pm
Tuesday 10am – 1:30pm
Wednesday 1pm – 4pm
Thursday 1pm – 4pm
Friday 9am – 12pm
Patients must requalify every two years.
Bring Your Documentation
You will need to bring ALL of the following documents to Admissions:
- A valid photo ID
- Proof of income (recent paystubs, paid worker letter or awards letter)
- Latest filed 1040 Income Tax Return
